Tournament Application

 

Please complete and mail:
  1. Application Form :
  2. Roster Information Due by October 20th if Out of State Team. If a Colorado registered Team I need your Pointstreak Team designation numbers for porting purposes.
  3. Registration Check in the appropriate amount (minimum 4 game guarantee), $1250 for Midget, $ 1150 for Bantam and girls U-19 & U-14,   $1050 for Squirt and Pee Wee -- made payable to: “Vail Eagle Hockey Association” Send Application and Check to:

Deadline for application is October 15, 2011 (except squirt teams - dead line is October 22)– applications must be postmarked no later than October 13, 2011to be considered. Please be sure information is complete, correct and legible – program rosters will be taken directly from information submitted. Telephone, fax and e-mail information is especially crucial for accurate and timely communication. If you decide to fax or e-mail the application or (roster information out of state teams), please mail the originals along with your check. Teams are guaranteed a minimum of 4 pool play games – the top two teams will compete in the Championship game. Your entry check will be returned to you if you are not accepted into the tournament.
